441 Mpox in persons living with HIV: Results from an international dermatologic registry

In 2022, mpox virus spread globally with 99% of cases in non-endemic countries. People living HIV (PLHIV) are disproportionally affected, often more severe clinical features and outcomes. The AAD/ILDS Dermatology COVID-19, Monkeypox (mpox), Emerging Infections registry captured from 13 countries a de-identified REDCap registry. We aimed to examine cutaneous symptomatology outcomes PLHIV. Of 119 reported cases, 44 were PLHIV (35%). Cases 98% male, median age 38 years, located Europe (57%) the U.S. (39%). Nearly half skin lesions as their initial sign (45%), 43 (98%) during illness. primary lesion locations peri-anal (34%) genito-inguinal (34%). Co-infection other sexually transmissible infections (STI) was common PLHIV, 57% vs. 38% all-registry (p<0.01). most co-infections gonorrhea, syphilis, chlamydia. Time resolution 17 days, 3 days shorter than which may be due higher use Tecovirimat (36% 25% cases; p<0.01). There no differences frequency hospitalization or scarring. One death reported.
